THE PROGRAM IS SIMPLE

Our Patient Fulfillment Program allows you to approve your patients’ account for their online purchase of Restorative Formulations. This helps your patients receive the high quality supplements you recommend and provides you with a monthly rebate.

1. If you have an account, you’re already registered.
   Approved Restorative Formulations accounts are automatically opted into the Patient Fulfillment Program. To opt out, call our office.
2. You give your patients an authorization code.
   It couldn’t be simpler. The code is your phone number.
3. Your patients register and order at www.restorative.com
   When they register using your authorization code, they are linked to your account. They receive Free Shipping for orders over $75 USD/$98 CAD.
4. Orders are shipped directly to your patients.
   We suggest you dispense each patient’s first Restorative Formulations products in your office, then let them place later orders online.
5. Every Monday you receive an email of what your patients are ordering. You receive a monthly report of your patient’s purchases along with a check for the prior month’s patient purchases.
   Our program allows you to choose your rebate amount and/or the discount you pass to your patients.
